Inspector Pulls Mambo Italiano Permit for Repeat Violations

The popular restaurant closed briefly, but reopened Thursday after compliance hearing. Caribbean Fiesta cited for several infractions - other scores range from 84-B to 98-A.

Following is a list of five Peachtree Corners restaurants which have been inspected recently by the  Gwinnett County's Board of Health.

Inspectors from the Board inspect area restaurants every six months and then grade them based on extensive criteria that include everything from food being stored at proper temperatures to employee hygiene.

Restaurants are required to display these reports in a prominent place for diners to view. This is what we found in a random search of inspections on the county's website.

Name: Mambo Italiano (*Permit reinstated after suspension for repeat violations)
Location: 5165 Peachtree Parkway
Inspection Date: Jan. 8, 2014
Grade: 72-C
Inspector's Comments: 

  • Observed multiple hand sink knobs with built up debris. All knobs and skins cleaned. Hand sinks must be cleaned as many times as necessary to keep fixtures clean.  Corrected On-Site.  New Violation.
  • Observed no hand soap at hand sink. Soap provided. Each hand sink shall be provided with hand soap.  Corrected On-Site.  New Violation.
  • Observed no paper towels at hand sink. Paper towel provided. Each hand sink shall be provided with paper towels or approved hand drying device.  Corrected On-Site.  New Violation
  •  Observed multiple cold held potentially hazardous foods (PHF) held greater than 41oF. Food discarded. All cold held PHF shall be held at 41oF or less.  Corrected On-Site.  Repeat Violation.
  • Observed mold on wall at dish machine. Floors need cleaning. Physical facility shall be cleaned as necessary to be maintained in a clean condition.  New Violation.
  • * Permit suspended for 3rd repeat violations of 6-1A and 6-1C. compliance conference scheduled on 1-9-14 at 11:30 am at Gwinnett County Environmental Health office. Notice of permit suspension given to manager. A risk control plan must be completed for 6-1A and 6-1C by permit holder. Other violation must have a corrective action plan prepared by permit holder. These plans and any supporting paperwork must be brought to the meeting. Fees may be applicable if additional inspections are required.


(After a third repeat violations, the owners voluntarily closed the restaurant and re-opened following a compliance hearing Thursday morning.)

Grade: 81-B
Inspector's Comments: 
  • Employee returned from the restroom to the food service area and resumed activities without washing hands in the hand-washing sink. Employee was told that he needed to wash his hands. Employees much wash their hands after returning from the restroom and before resuming food service activities. Washing hands at the lavatory is not enough. A second hand washing must be done once returning to the food service area. Employee washed his hand at hand sink in the food service area.  Corrected On-Site.  New Violation.
  • Food debris was on the can opener. It was stored in the holder. Wash the can opener before returning it to the holder. New Violation.
  • Light fixture cover in the rear food prep and storage areas is damaged leaving some light bulbs unshielded. Replace the damaged light fixture cover.
  • New Violation.
  • The exhaust fan in the men's restroom is not working. Replace the exhaust fan with one that works.  Repeat Violation.
  • Grease filter has part of it missing leaving an opening that allows grease to go up the duct in the exhaust hood. Replace the damaged grease filter and clean the duct.  New Violation.

Inspector's Comments: 
  • Papertowels not provided for hand sink located at bar area. Papertowels provided at time of inspection. Must ensure all handsinks are provided with papertowels.  Corrected On-Site.  New Violation.
  • At beginning of inspection, observed container with ice cream scoop being stored on back kitchen hand sink. Container and ice scoop removed. Must ensure hand sinks are only used to wash hands.  Corrected On-Site.  New Violation.
  • In-use utensils for potentially hazardous foods not being stored properly. Observed scoop used for ice scream being stored in standing water. Ice scoop removed. Must ensure all in-use utensils are properly stored.  Corrected On-Site.  New Violation.
  • Single-use gloves used for multiple tasks that present risk for cross-contamination. Observed employee handle dirty dishes and then handle clean dishes (at dishwash station) with same pair of gloves. Employee was corrected and removed gloves and washed hands. Must ensure gloves are changed when becoming soiled to help prevent cross contamination.  Corrected On-Site.  New Violation.

Inspector's Comments:
  • Observed employee placing onions on cooked fish without using an approved barrier.Fish discarded. Bare hand contact is prohibited when handling ready to eat foods.  Corrected On-Site.  New Violation.
  •  MSG,salt and sugar containers at chef's cart at wok area,not being ID with common name.Foods properly ID with common name.All foods when removed from original container,must be labeled with common name.  Corrected On-Site.  New Violation.
  • In-use, wet wiping cloths not stored in an approved sanitizing solution of 100 ppm available chlorine.Solution calibrated to 100PPM chlorine residual. Monitor chlorine concentration of sanitizer buckets in order to maintain approved chlorine concentrations.  Corrected On-Site.  New Violation.Ceiling tiles at dish machine area with mold accumulation.Increase cleaning frecuency of ceiling surface to avoid the mold build-up.  New Violation.

Inspector's Comments: 
  • Wiping cloths stored in a container that did not have an adequate concentration of sanitizer. Sanitizer solution changed out. Monitor sanitizer concentrations to make certain they maintain adequate levels (50 - 100 ppm if chlorine, 200 - 400 ppm or manufacturor's recommended level if QA)  Corrected On-Site.  New Violation.
  • Single use cups in sleeve not stored a minimum of 6 inches off of the floor. Make certain all utensils and food-contact surfaces are stored a minimum of 6 inches (15 cm) off of the floor.  New Violation.
  • Light bulb on front of hood needs shielding. Install sheild.  New Violation.

  • Lid of deep freezer damaged and held together with duct tape. Materials used in the construction of food equipment must be safe, durable, corrosion-resistant, and nonabsorbent.  Repeat Violation.
