The charges stem from a Dec. 1, 2011 incident in Quincy after state police were dispatched to the scene of a motor vehicle accident by the ramp on Burgin Parkway. The charges stem from a March 19, 2011 incident in Braintree after police responded to a report of a woman passed out at the wheel of a vehicle at the intersection of Cottage Avenue and Quincy Avenue. According to police, Reis informed them she recently had her big toe cut off and was having nerve problems in her left leg. The charge stems from a June 6 incident in Braintree at 9:45 p.m. when police were dispatched to 3 McCusker Drive for a report of a fight with screaming heard in the background of a 911 call, police said in the incident report.
