The Pima County Health Department needs your help defining its vision and mission, and setting goals to align the Health Department’s resources with the community’s public health priorities.

Your responses to this short survey will help the Health Department improve programs, services, and more to improve the health of all members of our community. There are no wrong answers, so please be frank and forthcoming when filling out the survey.

The survey is being conducted by an outside agency, so the information provided will be anonymous and the opinions expressed will be reported in aggregate only.
