San Francisco Free Clinic, LifeLong Medical Care & La Clinica de La Raza Receive $125K in Grants from CVS Health Foundation
Three Northern California Health Organizations receive Grants from the CVS Health Foundation to Improve Access and Affordability
March 30, 2016 – The CVS Health Foundation today announced that LifeLong Medical Care has received a $50,000 grant , San Francisco Free Clinic has received a $25,000 grant, and that La Clinica de La Raza has received a $50,000 grant in partnership with the National Association of Free & Charitable Clinics and the National Association of Community Health Centers . The grants have been awarded as part of the CVS Health Foundation’s multi-year, $5 million commitment to address the need for more accessible, coordinated health care in local communities.
LifeLong Medical Care’s grant will support the implementation of an organization-wide approach to panel management for adult patients with heart failure, who are at high risk for poor health outcomes and high cost of care. LifeLong Medical Care will also pilot a process to monitor medication refills and improve current timeliness.
La Clinica de La Raza will utilize the funding to support the Joint Venture Health Program, an innovative health coaching initiative that trains student nurses, psychology students and medical students to work as health coaches in a care team to provide expanded services to patients living with chronic diseases.

The grant from the CVS Health Foundation to the San Francisco Free Clinic will support the expansion of a full-service evening clinic which will increase overall patient appointment capacity by 5 percent , including increased access to working patients, who might otherwise not be able to receive health care services during regular business hours.
