Whooping Cough Booster to Be Offered at Calabasas High

The Tdap booster is mandatory for all students who are entering grades 7 through 12.

A new California law requires all 2011-12 students entering seventh through 12th grades to be immunized with a pertussis (whooping cough) vaccine booster called Tdap.  

The now requires an updated Documentation of Tdap Administration form to be submitted to your school's health office before June 16 to ensure that the student can start school on Aug. 24.

If your child has already received the Tdap vaccine, please submit documentation to your school's health office as soon as possible. For students, you can print out the attached form by clicking here, have your doctor complete it, then send it to the school health office by e-mail (to Health Clerk Joey Levin at Jlevin@lvusd.org) or fax (818-591-0276). 

Note: Forms must be sent to your child's current school, so that it can be included with their health records. If your child is currently in eighth grade,  send the form to the health office at his or her middle school.

If your child has not received the new Tdap vaccine, you should make arrangements to have it done as soon as possible, then complete and submit the form. 

Calabasas High School, in collaboration with Gelson's Pharmacy, will be holding  Tdap (pertussis) vaccination clinics at the school on the following dates:

  • Wednesday, May 18; 1:30 p.m.-6 p.m. (Calabasas High School, Student Room)
  • Saturday, June 4; 8 a.m. to noon (Calabasas High School, Student Room)

To sign up for one of these dates, contact Paloma Felisberto, Calabasas High Coyote Club, at paloma@macint.com; provide your student's name, birth date, grade and requested date of clinic. The cost of the vaccination is $70, less than many health care providers charge. A portion of the proceeds from the clinics will go toward supporting Calabasas High athletic programs.

The clinics are open to Calabasas High students and middle school students. Adults and college-bound students may also want to get this vaccine. Walk-ins are accepted. Everyone must complete a registration/screening form prior to the vaccination. 

Upon completion of the vaccination, Gelson's will provide the appropriate documentation to your school's health office (Calabasas High, A.E. Wright, A.C. Stelle).
