FREE Diabetes Health Fair This Saturday 11/7/15

THIS SATURDAY the Diablo Chapter of the California Society of Health-System Pharmacists (DSHP) is partnering with Novo Nordisk to put on an exciting opportunity for your patients.

What: FREE Diabetes Community Outreach Event (The event is FREE and open to the public)

Where: John Muir Medical Center, Concord (Concord, Martinez, and Kunkel rooms)

When: Saturday, November 7, 2015 from 11am-3pm

At the Free Diabetes Community Outreach event there will be:

· Dental, foot, hearing and eye screenings

· Physical therapy

· Brown bag medication reviews

· Educational mini lecture series

· Smoking cessation education

· POC glucose and blood pressure readings

· Celebrity chef cooking demonstration

· Lunch provided
