Meals on Wheels Contracts With Agency To Serve Patients With Chronic Conditions

The move boosts the Contra Costa County service's goal of reducing hospital readmissions.

CONTRA COSTA COUNTY, CA - Partners in Care Foundation has contracted with Meals on Wheels and Senior Outreach Services (MOWSOS) to provide social supports for high-risk patients with chronic conditions in Contra Costa County.

Partners is the lead agency in the Partners at Home Network (Partners), a statewide network of community-based organizations (CBOs) providing coordinated social services to vulnerable people across the state.

The Network includes both public and private social service organizations in 16 different California counties. MOWSOS is the contracted provider in Contra Costa County.

The goals of the collaboration are threefold:

1. Reduce hospitalizations through evidence-based care coordination

2. Improve health outcomes by addressing the social determinants of health

3. Reduce health care costs

Commenting on the contract, June Simmons, Partners’ CEO said, “We’re delighted to welcome MOWSOS into the Network. They provide an essential service to a vulnerable population, and further broaden the Network’s ability to deliver comprehensive, continuous, quality care to older adults statewide.”
