Fake Pharmacist Filled Prescriptions At Fremont Walgreens: Report
The woman worked at 3 different Walgreens — in Fremont, Milpitas and San Jose — and all 3 could lose their pharmacy licenses.

FREMONT, CA — A woman who was once a licensed pharmacy technician but never a licensed pharmacist signed off on or dispensed 745,355 prescriptions at Walgreens stores in Fremont, Milpitas and San Jose, according to reports. She is no longer employed by Walgreens, but the company's use of an unlicensed pharmacist could lead to the revocation or suspension of pharmacy licenses for those stores. The Fremont Walgreens implicated in the investigation by the state Board of Pharmacy is located at 2600 Mowry Ave. The Milpitas location is just over the Fremont/Milpitas border at 1833 Milpitas Blvd.
The woman accused of posing as a pharmacist is Kim T. Le. Le allegedly used the pharmacist license number of another pharmacist with the same name who was not employed by Walgreens.
“Upon learning of this issue, we undertook a re-verification of the licenses of all our pharmacists nationwide to ensure that this was an isolated incident,” said Walgreens spokesman Jim Cohn in a statement.

State pharmacy inspectors uncovered the alleged deception during a routine visit to the Fremont Walgreens store. The inspectors found discrepancies and zeroed-in on Le. She once had a pharmacist technician license but it expired in 2008.
The third pharmacy listed in the complaint is located at at 2105 Morrill Ave. in San Jose.
