Want to stay active and independent for life? This chair-based SAIL (Stay Active and Independent for Life) exercise class is led by a Certified instructor and uses seated exercises to strengthen the body, improve balance and flexibility, and help reduce the risk of falls.
CHAIR EXERCISE CLASSES RELEASE AND WAIVER
You are registering to participate in chair exercise classes sponsored by Independence at Home, a SCAN Community Service (SCAN). These chair exercise classes are not part of any health care benefits provided by SCAN.
By logging into this class, you have self-elected to join this class. By staying and participating in this class you waive and release SCAN and all members of their team, from any claims of negligence, implied or otherwise, of personal injury sustained by you in connection with or arising out of or resulting from participation in the chair exercise classes.
By participating, you understand that there may be certain health and other risks inherent in participating in a chair exercise class and that we advise you to consult a physician as to the risks and whether you should participate in chair exercise classes.
By participating, you further understand that SCAN has not made any representations as to whether it is advisable for you to participate in the chair exercise classes and does not do so, impliedly, expressly by accepting your application or by allowing you to participate in the chair exercise classes.
