Advance Health Care Directive: Protecting Yourself & Your Family - Information at the Contra Costa County Public Law Lib

Contra Costa County Public Law Library - Your first stop for legal information

An Advance Health Care Directive (AHCD) gives power to a person you select to make health care decisions for you, ONLY IF you can’t speak for yourself and gives written instructions about your health care. It will save your family from having to make very difficult decisions about your care in times of crisis. Everyone over 18 should have an AHCD, which can be changed at any time by preparing a new form.

The Contra Costa Public Law Library, with branches in Martinez, Richmond and Pittsburg, has information to guide you in preparing an AHCD, including books written for the non-lawyer, such as: “Living Wills & Powers of Attorney for California,” which explains:
