Patient Advocate Senior Screening Program

ATTENTION HEALTHCARE PR0VIDERS;

 

Family Peace Inc., 501 (c)(3) http://familypeaceinc.org/alzheimers.html; is a non-profit organization launching our; "Patient Advocate Senior Screening Program"; (PASSP), concurrently with the Alzheimer Foundation of America  (AFA) National Memory Screen Day, November 13, 2013.

We understand that physicians are overwhelmed; and these screenings can be time consuming and frightening in a physician’s office, (and it is most certainly seldom, that the senior patient participates in these screenings in an expeditious manner).

These non-medical visits can be “referred" to Family Peace Inc., 501(c)(3); and the treating physician will receive results; from a certified Patient Advocate, (prior to office visit); and will be able to bill under Medicare and Medicare Advantage plans.

 

We know health-care providers; mutually support Family Peace Inc., 501 (c)(3)’s  value of theses  processes, and our compassionate Patient Advocates allays some patient's fear and effectively reports any signs that show that there is a potential problem; (and if requested can provide a "non-medical safety assessment").

 

In any case, Family Peace., Inc., 501(c)(3), takes a compassionate proactive approach to raise awareness about mental health issues and to begin a conversation about possible problems, symptoms and treatments.  

 

Great News! – As part of Medicare Annual Wellness Visit – we provide a Mini Mental Status Examination (MMSE), which include memory screenings that have been around for years – but seldom used in a family practice office – until now

 

Through advocacy by many AFA members including; Family Peace Inc., 501(c)(3); the wheels of government have turned.  “What started as a grassroots effort has gone to Washington, been heard, caused a major change in procedure and has come back to our own community”; quoted from Septembers newsletter of AFA 2012.
