The following applies for those ages under 64 who purchase Individual or Families plans not through an Employer:
If you Qualify for ACA (AKA. Obamacare Tax Subsidy) you have until 12/23/13 to pick a plan, if you do not qualify for any Tax subsidy you have until 12/15/13 to start a plan January 01, 2014!
The only PPO Obamacare plan that will accept Hoag in 2014 is Health Net under the highest deductible ($5000), please note that if you purchase a Anthem Blue Cross EPO plan they will pay "ZERO" if you use them for any "non Emergency Services" in 2014!!!..
