Keep Eye Out: Man Missing From Care Facility On Mt. Diablo Boulevard

BREAKING: The 71-year-old suffers from dementia.

CONTRA COSTA COUNTY, CA – A 71-year-old man with dementia is missing today after walking away from his care facility in Walnut Creek.

Walnut Creek police said that Yang Chang was last seen at 5:30 a.m. at his assisted living facility on Mount Diablo Boulevard wearing a white T-shirt and khaki pants.

Chang has memory issues, but is otherwise healthy and has a history of walking away from care homes over the past year. Less than a week ago, he was relocated from a care facility in Oakland to the Walnut Creek location, so police said it is possible he may try to travel back to the Oakland area.

Chang is described as an Asian male, 5 feet 8 inches tall, 155 pounds with gray hair and brown eyes.
