This program helps adults of all ages prepare for medical decisions about care near the end of life providing information on advance health care directives ideas for talking with family members and choosing a healthcare decision-maker. Information is included about a California law that helps patients and their doctors discuss and document treatment choices in a Physician Orders for Life-Sustaining Treatment(POLST). Program presented by The Sacramento Area POLST Coalition
