REGISTRATION REQUIREMENTS :
1) Participants must be present at sign- ups to be weighed in by a board member and must be accompanied by a parent or legal guardian to register.
2) Participant's original birth certificate AND 3 copies.
3) Two types of proof of residence is required. Example: SDG&E, water or cable bill.
4) Pictures will be taken at Registration.
5) Sports physical prior to July 23rd, 2011 - no physical, no practice
6) Last quarter report card must be turned in by equipment handouts. (No report card, no equipment).
IMPORTANT: You must have all paperwork mentioned above turned in prior to first day of practice (end of July) or your child will not be eligible to practice. No exceptions will be made.
