Service providers are partnering with Alameda County Health Care Services Agency’s Getting the Most Out of Life hospice coalition to engage households in the difficult subject of end-of-life care and aging.

Join The Conversation… The Conversation is about what matters to you, not what’s the matter with you. Our purpose is to encourage people to consider, communicate and document their wishes long before any medical crisis occurs. The Conversation is personal not medical. Hear more.

Participants experience a practice-based training module so that they will:

1) See an advance care planning conversation demonstration;

2) Practice having an advance care planning conversation;

3) Receive a Conversation Starter Toolkit;

4) Learn about the forms (tools) for Advanced Care Planning documentation;

5) Receive POLST and Advance Health Care Directive forms;

6) Receive an invitation to work with the campaign and have The Conversation with co-workers, family and friends;

7) Last, work with us to celebrate the National Healthcare Decisions Day.
