Vaccine
Required Doses
Polio
- At least three doses.
- If last dose was given after the 4th birthday, one more dose is required. (A total of 4 is sufficient given anytime.)
Diphteria, Tetanus and Pertusis
- Children 6 and under: Five doses at any age
- Age 7 or older: At least three doses. If last dose was given before the second birthday, one more (Td) dose is required. (A total of four is sufficient given anytime.)
- Grade 9 and higher: four doses
Measles, Mumps, Rubella (MMR)
- Two doses on or after first birthday
- Grade 9 and higher: One does on or after first birthday
Hepatitis B
- Three doses at any age
- Students age 11-15: Either two-dose formulation (Adult) or three-dose formulation
Varicella (Chicken pox)
- Kindergarten: One dose
- Out of state entrants (Grades 1-12): One dose for children under 13 years; two doses if immunized on or after 13th birthday.
