Nursing Home in Danbury Fined: Report

The state Department of Public Health fined four nursing homes for various violations, including sending home a resident with the wrong meds

DANBURY, CT — Western Rehabilitation Care Center in Danbury was fined $10,000 following several incidents, according to the Connecticut Health I-Team.

In November 2019, a licensed practical nurse (LPN) discharged one resident with another resident’s medications. The error was realized three days later, after the resident had grown increasingly drowsy, according to the state Department of Public Health.

Later that same month, a resident left the facility for five days, returning only after police issued a Silver Alert and found the resident at home.

Later in December, the DPH found that annunciators, which notify staff of an alarm, were faulty when an alarm sounded for 12 minutes before a staff member silenced it.
