Updated Database Reveals Patient Deaths, Violations in CT Hospitals: Report

The violations were found at the time of inspection at Connecticut hospitals and was prepared by the Connecticut Health I-Team.​

Several patient deaths, including the death of a newborn who was given an overdose of medication, were among various violations found at Connecticut hospitals, according to newly released inspection reports from state health officials, the Connecticut Health-Investigative Team reports.

In the first of its kind, the C-HIT team earlier this year compiled a massive database of every hospital in Connecticut and includes reports of violations found during inspections.

Now, the C-HIT team has just updated the database, which includes inspections at hospitals in Connecticut from late 2015 through April of 2016.

The new updated reports include information on some patient deaths at hospitals and some other patients who suffered injuries, according to C-HIT.

In this database you can read the entire inspection report and violation along with the hospital’s corrective action. The inspection reports and violations date back to 2013-2015.

In the newly updated reports, C-HIT states several violations were found at Yale-New Haven Hospital including a case in July 2015 where a newborn died after being given too much amiodarone. Too much medication may have been administered due to a “default frequency” that was automatically entered into the computer system, C-HIT reports.

A patient at Yale-New Haven Hospital also died after having a heart attack during an eye surgery procedure in January 2015, C-HIT reports. Also a patient died at Norwalk Hospital after possibly overdosing on medication in April 2012, C-HIT reports.

Numerous other violations were found at hospitals in Connecticut.
