Reminder: Health Department Survey for Residents

Attention all Madison Residents: Now is your opportunity to get involved! The Madison Health Department encourages all residents to participate in our community health assessment. The assessment will help us gather accurate information representative of our community and identify issues for action to help improve the overall health and well-being of our residents. The survey will be available through the month of March, please fill out the online assessment (see link below).
Alternatively you can complete a paper copy at the following locations:

• Madison Health Department located at the Madison Town Campus, 8 Campus Drive
• Scranton Memorial Library, 801 Boston Post Road
• Madison Chamber of Commerce Office, 12 School Street
• Madison Senior Center, 29 Bradley Road
