The Town of Madison Health Department is pleased to announce that Griffin Health will be holding two Vaccine clinics in mid-December 2021.
The vaccine clinics are for ages 12 and up.

Choosing to get vaccinated is an additional powerful tool in our fight against the COVID-19 virus.
Clinic #1 Thursday, December 16th, 9 a.m. – 3 p.m. Madison Town Gym, 6 Campus Drive. [RESERVE TIME]
Clinic #2 Friday, December 17th, 2 p.m. – 6 p.m. Polson Middle School Gym, 302 Green Hill Road. [RESERVE TIME]
Please bring a health insurance card and photo ID if you have them, but it is not necessary to have health insurance or ID to receive the vaccine. No one will be turned away for lack of insurance or ID.
