Community Health Survey Launched In New Canaan
The town's health department has collaborated with Norwalk Hospital and the Western Connecticut Health Network on the survey.
Town of New Canaan:The Town of New Canaan Health Department has collaborated with Norwalk Hospital and the Western Connecticut Health Network to conduct a New Canaanhealth needs assessment. This information is vital to the development of our Community Health Improvement Plan (CHIP). As part of this process, the Western Connecticut Health Network is conducting a short community survey to learn more about you and our community's health needs. Your voice matters to this process. The survey will take less than 10 minutes to complete. Thank you for your participation.
