North Branford Residents Urged to Take Health Survey
The survey inquires about health and health habits of the residents of Branford, North Branford, Northford and East Haven.

The East Shore District Health Department will be concluding the collection of Community Health Assessment Surveys from the public in 10 days. The information from these surveys will be combined with State and National data in the form of a report.
This Community Assessment Report will help the Health Department and its Community Health Improvement Board in selecting programs to create a healthier community.
The survey inquires about health and health habits of the residents of Branford, North Branford, Northford and East Haven in areas such as healthcare access, nutrition and physical activity. The surveys are anonymous and secure, which means they will only be used only for by the health department and for only this purpose.

If you fill out a survey you can stop by the Health Department and get a prize.
The East Shore District Health Department wants to thank all of the people who took the time to fill out surveys and will be presenting the final report in the the form of Community Conversations to be held in June.
