Patricia Adams, RN, MPH, CMC, administrator of McLean Home Care & Hospice will be speaking on “Ways to Stay Well at Home” and “Planning for the Future.” Pat is a highly experienced RN leader in home health care, as well as a certified geriatric care manager. She will share information about McLean’s “Living with Heart Failure” and “Breathe Easy (COPD)” programs. She will also share how a McLean Care Consultant (care manager) can help seniors and their families plan for the future, rather than waiting for a crisis to react.
