Center for Healthy Aging receives Social Accountability Award

Center for Healthy Aging, Hartford HealthCare, Leading Age, Social Accountability Award

Hartford HealthCare Center for Healthy Aging recently received the Social Accountability Award of Excellence from LeadingAge Connecticut in recognition of the Center’s work helping seniors and families/caregivers navigate Connecticut’s healthcare system. The award was presented on Oct. 24 during the LeadingAge Connecticut Annual Meeting held at the Aqua Turf Club in Southington.

Wendy Martinson, RN, MSN, program director of Hartford HealthCare Center for Healthy Aging, accepted the award. Established in 2004, Hartford HealthCare Center of Healthy Aging, a resource and assessment network for older adults and caregivers, now offers locations in two skilled nursing facilities, three family health centers and five hospitals.

LeadingAge Connecticut is a member of LeadingAge, a national association of 6,000 not for profit organizations dedicated to advancing policies, promoting practices and conducting research to support and empower people to live more fully as they age. The Connecticut membership includes 130 not for profit provider organizations, including Hartford HealthCare Center for Healthy Aging.

LeadingAge presents the Social Accountability Award of Excellence to recognize exceptional programs within member organizations.

“The LeadingAge Social Accountability Award of Excellence demonstrates Hartford HealthCare Center for Healthy Aging’s dedication to and stewardship of our older adults, families and caregivers,” said Lisa Connolly, NHA, MS, vice president, Hartford HealthCare Community Network. “This recognition verifies the Center’s extraordinary care and commitment to not only individuals but the greater community.”
