SOUTHINGTON, CT — Southington police on Friday released a report on a fatal machine accident at a local residence a week ago.

At 10:03 on April 17, Southington police received a 911 call from a Rockwood Drive residence about an accident involving a stump grinder, according to a report.

An investigation revealed that Carlos Vazquez, 64, of Bethany, was contracted by the homeowner to remove stumps from the property, according to a report. While using a stump grinding machine, Vazquez walked around to the front of the machine, where he lost his footing and fell into the blade, which as the time was still running, a report indicates.

Vazquez suffered fatal injuries and was pronounced dead at the scene, according to a report.
