New Hartford HealthCare Center for Healthy Aging opens in Newington

NEWINGTON – As life spans increase, greater numbers of individuals need new services and resources to help them age well. The U.S. Census Bureau projects that by 2060, 92 million Americans will be 65 or older.

To meet the need of older adults and families, in 2004, Hartford HealthCare developed a groundbreaking Center for Healthy Aging model. Now, 13 years later, Hartford HealthCare Center for Healthy Aging has eight locations where hundreds of families are connected with ideal support systems to assist their older loved ones in achieving an optimum quality of life.

In July, one of the newest locations opened in Newington at Jefferson House, located at 1 John H. Stewart Drive. Jefferson House, a department of Hartford Hospital, offers short-term/outpatient rehabilitation, skilled nursing and palliative care.

Michelle Wyman, LSW, CDP, is the dementia specialist for Hartford HealthCare Center for Healthy Aging at Jefferson House. She provides home, telephonic and office assessments specifically for individuals, family members and caregivers that are affected by Alzheimer’s disease or a related dementia. Effective strategies include- individualized care plans, coaching, education and connections to community resources and supports to ensure the individual has the supplemental services to help them live safer, healthier lives.

“I feel blessed to work in collaboration with such wonderful caregivers, both families and professionals, who strive to help the person with dementia live life to the fullest,” Wyman said. “Any education or support I offer to help with this continues to make my work fulfilling.”
