Enroll in a Local LifeStyle Change Program This Spring
Monitor My Health Offers Diabetes and Heart Disease Prevention at a Significantly Reduced Cost to Residents of West Haven

Monitor My Health, Inc., a Lifestyle Change Program based in Bridgeport, recently was recognized as the only nonprofit in Connecticut that currently participates in Medicare Part B funding and reimbursement. The program is FREE of charge for anyone 60 year of age or older who is prediabetic.
Dr. Dana P. Wade, MD, MBA, Founder and CEO of Monitor My Health, noted, “We are thrilled that our organization was recognized by the federal government for this prestigious statewide honor. Our non-profit is looking to establish new partnerships by offering more classes at program sites, including area public or private schools, businesses, churches, community and senior centers, and senior living communities.”
He added, “Our mission is to help people prevent diabetes, heart disease, stroke, and high blood pressure through screening and lifestyle coaching. In addition, we are reaching high risk populations in their efforts to prevent or delay type 2 diabetes."
This program is currently offered throughout Fairfield and New Haven Counties in the following communities: Stamford, Darien, Norwalk, Westport, Fairfield, Bridgeport, Milford, Stratford, Orange, West Haven, New Haven, and East Haven.
In 2018, Monitor My Health received its first federal scholarship. Due to the success of the first graduating class, this additional funding has been extended into 2019 and 2020.
In late 2019, Monitor My Health received its second federal scholarship. These reimbursed payments are part of a Value Based Payment System and are distributed after program participants successfully reach specific milestones.
This additional funding was issued by the Center for Disease Control and Prevention (CDC) and the CT Department of Public Health to offer a Lifestyle Change Program for diabetes and heart disease prevention at a significantly reduced cost to residents of Bridgeport, New Haven, and West Haven.
Monitor My Health is one of the few organizations in CT to receive CDC Full Recognition. This designation is reserved for programs that have effectively delivered a quality, evidence-based program and have fulfilled all the standards requirements and milestones for CDC recognition. The recognition status allows Monitor My Health to enroll in Medicare as Medicare Diabetes Prevention Program (MDPP) suppliers.
Diabetes and heart disease represent a major health problem and are a leading cause of death and disability in America. According to the CDC report of 2015, "30.3 million Americans – 9.4 percent of the US population – have diabetes and another 84.1 million have prediabetes. 90% do not know they are prediabetic."
"Since founding this not-for-profit in 2016, the region's need for a CDC-recognized Diabetes Prevention Program has increased dramatically,” said Dr. Wade.
In 2018 and 2019, Monitor My Health enrolled over 200 participants at high risk for diabetes and heart disease to lower their risk. All Lifestyle Change Program classes met for a year at 15 different community locations.
This past November, Monitor My Health held its first graduation ceremony at the Bridgeport Holiday Inn to celebrate the achievements of its program participants. The residents who completed the program had significantly lowered their risk for type 2 diabetes and heart disease and are not prediabetics anymore.
Dr. Wade is passionate about "transforming lives, making the impossible possible." Well-educated, he earned a Master in Business Administration (MBA) degree from the University of Bridgeport's (UB) Ernest C. Trefz School of Business with an impressive "straight A" (4.0 GPA) average. In addition, he was a Research Fellow in clinical cardiovascular disease from Albert Einstein College of Medicine and earned his MD from St. Matthews School of Medicine. He also holds a Bachelor of Natural Science and Mathematics from UB.
In May of 2019, he was inducted in the Delta Mu Delta, the International Honor Society in Business, Theta Delta Chapter at the University of Bridgeport.
In November of 2019, he was honored as an Ambassador of Peace by the Universal Peace Federation in Bridgeport. He was the only Medical Doctor to be recognized.
In December of 2019, he was recognized as the Hometown Hero by News 12 Connecticut.
