To help prevent the spread of the coronavirus, screening questions will be asked of everyone. Only one adult per family will be allowed in the clinic (unless there are two children ages 12 or under) and each person must wear a face covering.
A parent or guardian must bring the child's immunization record and accompany the child to receive services. If the parent is not present, written consent must be obtained prior to receiving services.
All new students in Pinellas County are required to have a school entrance physical, dated within 12 months of school enrollment.
