The sheriff’s office began investigating Levins on Nov. 2 after a doctor Levins worked with told deputies she was using his prescription pad to write bogus prescriptions for Oxycodone.

When detectives followed up, they discovered that Levins had written fake prescriptions for 10 patients between May and October. Most of the patients live in assisted living facilities and were unaware that the drug was being prescribed in their names.
