• Observed potentially hazardous food cold held at greater than 41 degrees Fahrenheit for more than 24 hours. Food item was discarded after brought to the attention of the PIC.
• Observed food being prep near dish washing area. Pic Was notified, who then told employee to prep in a safer more sanitary area.

• Several measuring cups and slicer observed with old food debris. Ice machine observed with slime growth on interior. CA: Discussed with manager cleaning procedures.
• Observed toxic item stored by food.
• Ice machine missing part of the top, using wood to cover hole/gap. CA: Told manager ice machine must be repaired using approved materials.
• Several containers holding utensils observed with old food accumulation. CA: Discussed cleaning frequency with manager
• Observed large number of fruit flies in dry stock area around onions. CA: Discussed removing over-ripe fruits to prevent flies.

• Observed raw animal foods not properly separated from each other in holding unit/during preparation. Chicken was placed above beef. PIC was notified of the issue and restacked shelves to put chicken in proper place.
• Observed dirty shelves, with much soil build up on it. Notified PIC who voiced that it would be cleaned.
