• Observed employee drinking from an open beverage container (Dasani water) in a food preparation or other restricted area. CA: Created a designated area for the drink and discussed with PIC about not having drinks in the prep area.
• Fried chicken, and beef found stored on shelves uncovered and subject to contamination.
• Observed raw/under-cooked animal foods, chicken, being prepared in fruit and vegetable sink. CA: Discussed with PIC who then voiced that the sink was used for multiple preparations. PIC was then informed on the proper way to use a prep sink, and how it is a requirement to use the sink for only one kind of prep.
• Observed personal items, cell phone and headphones, in area where food is being prepped. CA: Discussed with PIC about having a designated area for these item.
• Observed live flies in kitchen. PIC was notified and voiced that there would be a pest control professional coming to treat the facility.

• Worker seen handling ready to eat food biscuits, with bare hands when glove or utensils use is required. CA: Discussed with the PIC about proper use of gloves at the appropriate times.
• Observed buildup of soil in the interior of ice machine. PIC was notified of the issue and voiced that it would be corrected in the future.
• Observed no hand washing sign provided at a hand sink used by food employees.
• Observed pots, pans and containers being stacked wet. PIC was notified and discussed proper way of stacking to allow air circulation, such as staggering pans, or stacking in a pyramid formation. Repeat violation.
