• Observed potentially hazardous food (chicken and hot dogs) cold held at greater than 41 degrees Fahrenheit. CA: Discussed with PIC about checking temps and ensuring food remains at 41 Fahrenheit or below.
• Observed live flies in kitchen. CA: Discussed with PIC about pest control procedures and was to reduce the amount of flies in the kitchen area
• Remarks: Although pest control devices were in place, the frequent opening and closing of the back door in close proximity in the kitchen may be the reason as to why there was an excessive amount of flies in the kitchen. Also PIC was questioned as to why the chicken patties and hot dogs were out of temp, and no definitive conclusion was reached.

• Observed utensils (tongs) laying in food (vegetable mixture of cabbage carrots, and broccoli). CA: The food was thrown out after the issue was made known.
• Observed soiled reach-in cooler gaskets. CA: Discussed with PIC about cleaning frequency of gaskets.
• Observed live flies in kitchen. CA: This issue was made known to the PIC and it was discussed the need to open the door less frequently to reduce fly activity in the kitchen.
• Remarks: Major issue noted in previous inspection were corrected, such as using the proper sink to wash vegetables, and using gloves when handling ready to eat food.

• Observed encrusted material flour on clean metal containers. CA: The issue was made know to PIC who then took dishes to 3 compartment sink to be re-cleaned. Corrected On-Site.
• Observed cell phone sitting in prep area. CA: Discussed with PIC about creating a designated area for personal items. Cell Phone was removed.
• Observed live flies in kitchen. CA: Discussed with PIC the need for controlling pests.
