• Ice machine in need of repair. Plywood being used as makeshift door. CA: Discussed proper repair with management.
• Exhaust fan in room 105 not working. CA: Discussed repair with manager.
• Floors, walls and ceilings show significant signs of aging. Recommended that the entire facility be thoroughly cleaned and repairs made to chips/holes in the walls, torn carpeting cracked/broken fixtures in the restroom including replacing many ceiling tiles.
• Observed furniture, carpeting etc that appeared to be dirty, torn and in disrepair. Recommended having everything cleaned and/or replaced.
• Mattresses excessively stained and dirty. Recommended replacing and/or thoroughly cleaning.
