- Observed personal drinks sitting on top of prep cooler. Open drinks were discarded. Corrected On-Site
- Employee restroom not supplied with soap or paper towels. PIC to restock immediately... Corrected On-Site.
- Observed raw chicken rushed out the door when the health inspector walked into the kitchen. Upon following the employee with raw chicken, it was observed that a bowl of raw chicken was placed outside and was being covered up by the employee for an unknown reason. The chicken was discarded and the PIC informed of what had happened. Corrected On-Site.
- Observed several severely dented cans that were on the shelf waiting to be used. Discussed with PIC and the severely dented cans were set aside to be returned to the vendor. Corrected On-Site.
- Observed raw animal foods stored directly above lettuce that was partially covered in the walk in cooler. Discussed with PIC about proper storage and separation of raw foods and fresh foods needing no cook step before being served to the consumer.
- Observed foods in the walk in cooler uncovered and subject to cross contamination. Discussed with PIC and foods were covered during the inspection. Corrected On-Site.
- Observed food contact surfaces that were dirty with excessive food accumulations and food debris that were in need of being cleaned and sanitized. Prep areas, counters, pots, pans, dishes. Discussed with PIC and pointed out the areas of concern.
- Observed the slicer and band saw in the meat prep room that had excessive food debris still on them and were both in need of being cleaned and sanitized.
- Observed several potentially hazardous foods requiring temperature control not being maintained at proper temperature. Cheese dip on the floor at 57 degrees F. Beef intestines in a pot at 57 degrees F. Foods were discarded. Discussed in detail with the PIC about proper temperature control.
- Observed rice was sitting on the prep table and not on a heat source at 121 degrees F. Cooked potatoes on the prep shelving and not on a heat source at 65 degrees F. Foods were discarded. Discussed in detail with the PIC about proper temperature control. Corrected On-Site
- Observed several potentially hazardous foods being stored in the walk in cooler that did not have the required seven day date marking on them. Discussed with PIC. Actual cook dates were placed on the foods. Corrected On-Site.
- Observed contamination of food in the walk in cooler that had water or condensation from the a/c unit dripping onto it. Discussed with PIC. Foods were discarded and relocated until the drip could be repaired. Corrected On-Site.
- Observed utensils/scoops in bulk food containers (rice, flour, sugar, etc) that are being used either do not have handles or are being stored with the handles in direct contact with the food. Discussed with the PIC.
- Observed single use containers being reused to store other foods (gallon jugs). Discussed with PIC. These containers were to be discarded. Corrected On-Site.
- As noted on previous inspections, the freezers and refrigerators that have the lids/doors that are falling apart and exposing wires and insulation must be replaced immediately. The replacement equipment must be of commercial grade.
- Observed bulk containers for chips that are grey cans and not food grade. Food grade containers for bulk chips are white in color as some of those in use in the facility. New bulk containers for chips need to be used to replace the grey ones. Discussed with the PIC.
- Observed excessive grease build up on and around fryers especially at the back of the fryers and on the floor and wall behind the fryers. Discussed with PIC.
- Observed the facility as a whole is in need of being cleaned, floors, walls, equipment, etc. Discussed with PIC.
- A follow-up inspection is planned for Jan. 18.

- As noted on previous inspections, roaches both dead and alive observed.
- Bed bugs observed in room 102. PIC notified.
- Unapproved pest control method used: powder pest control observed in all rooms observed.
- Most recent inspection report not posted for public view.
- Lights and fan not working in room 106 and 141.
- Exhaust fans of rooms 106 and 141 not working. Mold found on ceiling of bathroom in room 106 and 141.
- Mold seen on wall, ceiling, and door frame of room 102.
- Hole in bathroom door of room 102.
- Wall paper peeling and stained in rooms 102, 106, 141, 111.
- Carpets badly stained and in disrepair in rooms 102, 106, 111, 141.
- Outside vents located above doors not properly screened for most rooms.
- Draperies of all rooms badly stained and/or in disrepair as observed through windows from outside of facility.
- Mattress and box spring in room 141 badly torn and slashed.
- Bed Bugs found in mattress of room 102
- Mattress in room 111 badly stained.
- Concrete in multiple areas of sidewalks are cracked and broken.
- Wall outlets uncovered or in disrepair in rooms 102, 141
- AC and heat unit in room 102, does not have proper control panel or knob.
- Pool not in operation or permitted. Not drained entirely, broken tile surround, and gate not locked to prevent access.
