- Person in charge lacks awareness in key areas of risk factor control or good retail practices for establishment.
- Observed employee switch from working with raw food to ready-to-eat food without washing hands. Employee stopped before beginning work with other foods and told to wash hands, employee then had to be taught correct way to wash hands with soap.
- Returned/previously served food not properly segregated. Returned food and equipment from service area being stacked next to a clean prep table and clean utensil storage. CA: cart used for storage moved to dishwashing area.
- Observed old food stuck to clean dishware/utensils. Items removed. Chlorine sanitizer not at proper minimum strength for manual warewashing.
- Prepared food not marked with date of preparation or date for disposal. Labels only informed with day of the week, no date written, and no time established for items cooling or holding procedures.
- No handwashing sign provided at a handsink in restrooms.
- Observed interior of reach-in cooler soiled with accumulation of food residue.
- Observed build-up of grease on nonfood-contact surface.
- Observed build-up of food debris, dust or dirt on nonfood-contact surface.
- Observed food debris accumulated on kitchen floor.
- Observed wall and floor soiled with accumulated black debris.
- Observed attached equipment soiled with accumulated grease.
- Floors not maintained cleanable and durable. Broken tiles around drains hold water and debris

-Employee hygiene and training need to addressed immediately. Employee was seen not washing hands and did not know proper procedure for washing hands;
-Food contact, food storage, non-food contact, and equipment all needed to be better cleaned. Manager stated that cleaning is done once a week.

-Floor around drain missing tile and holding water, floor and walls around storage and hand sinks covered in black residue.
-Hand sink not solely used for hand washing.
-Different labeling procedure needed, current labeling not adequate with dates and times.
-Manager says he will have all problems fixed by Thursday, March 24. I suggested that he re train staff and thoroughly clean facility as well as fix areas of disrepair.
