• All rooms observed showed significant signs of wear and tear and many areas that are in need of cleaning including the toilets, tubs/showers and floors. Repeat violation.
• Floors, walls and ceilings show significant signs of aging. Recommended that the entire facility be thoroughly cleaned and repairs made to chips/holes in the walls, torn carpeting cracked/broken fixtures in the restroom including replacing many ceiling tiles Repeat violation.
• Observed furniture, carpeting etc that appeared to be dirty, torn and in disrepair. Recommended having everything cleaned and/or replaced. Repeat violation.
• Mattresses excessively stained and dirty. Recommended replacing and/or thoroughly cleaning. New violation.

• As observed in previous inspection, employees had open beverages and personal items in fridge or around food prep area. PIC adjusted accordingly once issue was brought to her attention.
• Observed chlorine sanitizer not at proper minimum strength for manual warewashing. PIC was notified and sanitizer was changed.
• As observed in previous inspection fish, mackerel, tuna, salmon, and snapper, in sushi bar not date marked. Rice was observed not being date marked. PIC was notified and advised personnel to immediately begin date marking.
• Observed no handwashing sign provided at a handsink used by food employees. PIC was notified and who then said that the signage would be posted on next inspection.
• Observed grease accumulated under hood of cooking grill. Discussed with PIC about cleaning frequency.
