• Observed potentially hazardous food (cream cheese) cold held at greater than 66 degrees Fahrenheit. CA: Discussed another container to store cream cheese in, cheese was discarded.
• Potentially hazardous food (sausage and eggs) not held at 135 degrees Fahrenheit or above. CA: Food items were discarded.
• Observed cups used as scoops in dry goods containers. CA: Informed PIC that approved scoops must be used, with handles sticking up above food.
• Remarks: Discussed and provided form for the employee health policy; discussed the use of time as a public health control and provided form; informed PIC that another ServSafe may be needed.

• Person in charge lacks awareness in key areas of risk factor control or good retail practices for establishment. CA: Persons in charge, managers or CFSM must be properly trained or retrained.
• The PIC could not provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. CA: Will have documentation on 4/16/2018.
• Establishment does not have established procedures, or clean up kit, for employees to follow and use when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food establishment. CA: Written procedures were given to PIC and clean up kit will be verified on 4/16/2018.
• Observed Lobster dip, a TCS Food, being held at 114 degrees. CA: Discussed monitoring food to ensure temperatures and food was discarded.
• Observed cell phone being stored over food. CA: Items were removed and put in another location.
