• Observed no paper towels located at handwashing sink in employee restroom. CA: Discussed with PIC about always having paper towels available.
• Observed Wiping cloth chlorine sanitizing solution not at proper minimum strength. CA: Discussed with PIC and Solution was remade to proper strength and temperature.
• Observed food container not properly labeled. CA: Discussed with PIC about the importance of labeling containers to be able to distinguish between substances.
• No handwashing sign provided at a handsink used by food employees. CA: Discussed about posting signs.
• In-use utensil not stored on a clean portion of food preparation or cooking equipment.
• Observed employee using random cup to scoop ice in to cups and containers. CA: Notified staff and PIC about the issue, and advised them to have one utensil to scoop ice instead of using what ever cup is nearby. Corrected On-Site.
• Remarks: Created a list for PIC that described all violations mentioned in the report above. Discussed the issue of repeat violations, and having them corrected them by the next inspection. Also, discussed with PIC about paying close attention to gaskets in the outdoor cooler, to avoid potential hold holding issues.

• Observed personal items including a cell phone and wallet of employee, sitting on prep space in kitchen. These items should be stored in designated area.

• Flower bin next to hand sink observed open with water drips all over surface from hand washing. CA: Discussed with employee keeping all food bins closed or away from sink areas.
• Two slicers observed with large accumulation of old food. CA: Employee said they weren't used, must be discarded from kitchen.
• Ice scoops stored on un-sanitized surface on top of ice machine.
• Ice machine observed with slime growth on interior. CA: Employee said they would be corrected.
• Stir Straws at bar not wrapped. CA: Discussed with manager about keeping straws behind bar and handing out individually.
• Employee medication and cell phone observed on rack holding food items. CA: Told employee that all personal items have to be stored away from kitchen area.
• Vent hood filters in dis-repair and large accumulation of grease. CA: Told employee they had to be replaced.
