  • Observed TCS foods (lettuce at 54°F, stuffing at 53°F) not stored at 41°F or below. CA: PIC discarded items because it was not determined how long items were stored above 41°F.
  • Observed employee coat hanging on clean dish storing rack in dish pit. Also observed employee phone stored on prep table. CA: PIC moved all items to designated area.

  • The PIC could not provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. CA: Informed PIC that documentation should be available in order to verify that all employees were informed to report certain food borne illnesses. PIC was given another employee health policy.
  • Observed no soap located at hand sink near the dish area and no paper towels located at the hand sink near the prep cooler area. CA: Informed PIC that soap and paper towels must be supplied at hand sinks at all times. PIC said they were in the process of putting up new paper towel dispensers.
  • Establishment does not have established procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food establishment. CA: Discussed the importance of having a bodily fluid clean-up kit that is readily available. Provided PIC with a procedure form.
  • Observed a container of raw chicken stored next to a bottle of ranch and a bowl of raw chicken stored above a container of marinara in the reach-in cooler. CA: Discussed and explained proper separation with PIC. Items were corrected.
  • Observed build-up on top interior portion of the microwave.
  • Discussed and provided forms for the employee health policy/bodily fluid clean-up kit.

  • Observed employee drink stored in grill prep area beside clean plates/utensils. CA: Drink was moved. Discussed eating/drinking with PIC.
  • Observed employee place raw fish on to grill, then later touch cooked fish without changing gloves first. CA: Discussed glove use with PIC. Employee changed gloves.
  • Observed TCS foods not held at 41°F or below (diced tomatoes 54°F, cheese sauce 53°F). CA: PIC discarded items, replaced ice, and brought out new food items. PIC chose to discard items although they were held for less than 4 hours and could have been rapidly cooled.
  • A follow-up inspection will be held on April 2.

  • Observed olives and lettuce in hand washing sink. CA: PIC removed food debris from sink. Explained to PIC that hand washing sink is only for hand washing.
  • Observed black mold/debris build-up on drink machine nozzles. CA: PIC cleaned nozzles.
  • Observed expired apples in front reach-in cooler. CA: PIC discarded apples.

  • No soap located at handwashing sink by the bar area. CA: Informed PIC that all hands sinks must be supplied with soap and paper towels at all times. Soap was refilled.
  • Observed no hot water at the front hand sink by the bar area. CA: Notified PIC and it was stated that work order had been put in and would be corrected today.
  • Observed raw chicken and beef not horizontally separated in a prep top cooler in order to prevent contamination. Observed raw chicken stored over mushrooms and spinach in reach-cooler. Observed raw chicken stored on top of a container of raw beef and also stored over a red sauce in the walk-in cooler. CA: Discussed with PIC about the importance of separating raw animal foods and ready-to-eat foods, items were rearranged.
  • Observed build-up on soda dispenser nozzles. CA: Informed PIC and it was stated that nozzles would be removed for cleaning.
  • Observed potentially hazardous food (chicken wings) cold held at greater than 41 degrees Fahrenheit. CA: Notified PIC and wings were discarded.
  • Potentially hazardous food (queso) not held at 135 degrees Fahrenheit or above. CA: PIC was notified, queso was reheated to 165 degrees, considering that it was still in the correct time parameter.
  • Observed cooked potentially hazardous food (pork) not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours located in the walk-in cooler. CA: Notified PIC and it was stated that food had been in walk-in cooler almost 24 hours. Food was discarded and proper cooling methods were discussed.
  • Observed food containers of flour, sugar, and sauce bottles not properly labeled. CA: Informed PIC that if food is removed from its original container then it must be properly labeled.
  • Observed broken floor tiles in the kitchen area as noted in a previous inspection. CA: Informed PIC that floors must be easily cleanable so that it does not harbor bacteria. Informed PIC that tiles should be repaired as soon as possible.
  • A follow-up inspection will be held on April 4.
