• Person in charge lacks awareness in key areas of risk factor control (he didn't ensure that staff checked concentration of machine ware washing frequently and could not locate employee health policy) or good retail practices for establishment. CA: PIC should receive more training.
• The PIC could not provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food.
• Chlorine sanitizer not at proper minimum strength for mechanical warewashing. CA: Chemical sanitizer was empty, and PIC changed the solution and sanitizer was at minimum strength. Corrected On-Site.
• Equipment and utensils not properly air-dried. CA: Discussed other methods and will come up with a plan to be implemented. New Violation. Must correct by July 3, 2017.
• Observed live flies in kitchen. CA: Discussed other methods with owner about fixing air curtain.

• Observed dead fly in salmon patty mixture in prep top. CA: Notified PIC and food was discarded. Corrected On-Site.
• After observing dead fly in salmon patty mixture, the PIC was advised to pay attention to openings --not leaving them open for long periods of time. There are insect control devices in place.
