• Observed employees (two servers) eating Mellow Mushroom pizza in a food preparation or other restricted area. CA: Food was removed from area after issue was being discussed.

• Observed old labels stuck to food containers after cleaning. CA: Informed PIC that labels must be completely removed before cleaning.
• Observed encrusted, soiled material on slicer. CA: Informed PIC that slicer needs to be cleaned thoroughly.
• Observed old food stuck to clean dishware. CA: Discussed with PIC about making sure dishes are clean after washing.
• Observed potentially hazardous food (salmon, tuna, smoked salmon, volcano sauce) cold held at greater than 41 degrees Fahrenheit. CA: Informed PIC that cooler is not cold holding properly, food was discarded.
• Observed dead roach in 3-compartment sink and in mop sink. CA: Notified PIC and he presented pest control records. Discussed the need to increase pest control measures.
• Remarks: Discussed and provided sheets for employee health policy/bodily fluid clean-up kit.
  Discussed with PIC about putting thermometers inside coolers. Will do a follow-up to see if cooler is fixed and holding at the proper temperature (Re-check was done on Oct. 16; sushi cooler was holding food at the proper temperature).

• Observed containers of broth stored on floor in walk-in cooler and boxes of food stored on the floor in walk in freezer. CA: Informed PIC that food items must be stored at least 6 inches off the floor.
• Remarks: Discussed and provided sheets for the employee health policy/bodily fluid clean-up kit. As noted earlier walk-in cooler fan was fixed onsite, will do a follow up to make sure items are holding at the proper temp.
