• Observed potentially hazardous food (chicken tenders and tomatoes, coleslaw, and lettuce) held using time as a public health control with no time marking. Food may not be served.

• Observed soil buildup inside ice bin. CA: Informed PIC that ice bin is in need of cleaning.
• Ready-to-eat, potentially hazardous food (chicken, cheese, turkey, ham) prepared on site and held more than 24 hours with not properly date marked(as noted in previous inspection). CA: Discussed date-marking with PIC.
• Observed no water in the public restrooms. CA: Pointed out to PIC and it was stated that issue will be corrected. Will do a follow-up within two weeks.
• Observed hole in ceiling in outside dry storage building and evidence of mice were present. CA: Discussed pest control measures with PIC, will do a follow-up to see if issue is taken care of.
• Discussed and provided forms for the employee health policy/bodily fluid clean-up kit and about using temperature logs. Will follow-up with PIC to see if pest control measures were taken and also to see if hole is sealed up in dry storage building.

• Observed employee with open sores on hands handling food. Food was discarded, employee washed hands and begin to wear gloves after being prompted to do so.
• Observed dented/rusted 4 cans (1 green bean, and 3 corn beef hash cans. Cans were moved to designated area, and employees were advised to monitor cans more carefully.
• Observed live flies in kitchen, In dish wash area. CA: This was due to the door being open and closed too frequently. Advised PIC to keep door closed more often.
• Although no violation was observed, proper raw animal food separation may be an issue due to limited reach cooler and walk in cooler space. Advised PIC to pay close attention and to not let certain raw foods touch nor to let raw and RTE foods touch.
