  • Person in charge lacks awareness in key areas of risk factor control or good retail practices for establishment / CA: Persons in charge, managers or CFSM must be properly trained or retrained .
  • The PIC could not provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food.
  • Observed employee drinking from an open beverage container in a food preparation or other restricted area. CA: Discussed with PIC and a new designated area was created.
  • Establishment does not have established procedures for employees to follow when responding to vomiting or diarrhea events that involve the discharge of vomit or fecal matter onto surfaces in the food establishment.
  • Observed improper horizontal separation of raw animal foods and ready-to-eat foods.
  • Observed old food stuck to clean dishware/utensils (glass pan in dray storage and large pan in main kitchen). CA: Items were rewashed immediately.
  • Observed sanitizing (chlorine) solution exceeding the maximum concentration allowed. CA: solution was diluted.
  • Plumbing system in disrepair.
  • Observed holes in wall being plugged by copper wire sponge pads. Also observed floor tiles missing and in need of replacement in hall way leading from main kitchen into dry storage.
  • Observed back door with noticeable gap at the bottom. CA: Discussed with PIC about having door replaced by next inspection set for June 6, 2018.
