- As noted on previous inspection, observed potentially hazardous food cold held at greater than 41 degrees Fahrenheit. CA: Food discarded and prep unit removed from kitchen.
- Cold holding equipment incapable of maintaining potentially hazardous food at proper temperatures.
- Failing prep top unit removed from kitchen. Before storing food in unit after being fixed or replaced, inspection is needed. Second unit is holding between 40 and 41*F. To keep unit from overloading and not holding temperature, PIC will keep a temperature log. Temperatures should be taken twice a day.

- Observed bread loaves with plastic torn open exposing the bread and a few pieces laying unprotected on the grey bakers racks in dry storage. Bread was discarded.
- Observed beef being cut and processed on the sink drain table that is clearly labeled for vegetables/produce. The meat was relocated to the meat prep sink and prep table during the inspection and the vegetable prep area was cleaned and sanitized. Discussed with PIC.
- Observed standing water and waste food debris on the "clean side" of the dish machine drain board (where the just washed dishes were sitting). Dishes were to be rewashed and the drain board cleaned and sanitized.
- Observed the 4th compartment of the manual ware wash sink being used to store utensils until they air dried but was dirty with waste food debris and did not appear to have been properly cleaned and sanitized prior to being used for air drying. All utensils were to be rewashed, rinsed and sanitized and the 4th compartment cleaned and sanitized as well.
- Observed the exterior and top of the dish machine that is excessively dirty with food debris and is in need of cleaning. Pointed out to the PIC during the inspection.
- Observed several potentially hazardous foods in the walk in cooler that were being held above the 41 Fahrenheit maximum cold holding temperatures: Deli Turkey - 43; Ham - 44; Boiled Eggs - 48. Foods were to be pulled and discarded. Discussed with PIC in the walk in cooler. Walk in cooler must be serviced to ensure proper temperature control is being maintained.
- Observed personal belongings (jacket) being stored in with the dry storage goods. Jacket was relocated during the inspection.
- Discussed with the PIC that an additional hand sink appears to be needed closer to the prep tables in the kitchen. Management needs to discuss and develop a plan and timeline for this additional sink to be installed.
- Ensure the all date marking is using the seven-day discard with day one being the cook day or the day the commercial packaging was opened.
- A follow-up inspection is scheduled for Feb. 17.

- Observed personal drinks and protein mix on prep cooler cutting board. Discussed with PIC and relocated.
- Observed mold growth on the inside of the ice machine. Pointed out to PIC.
- Observed the inside of the microwave having excessive food splashed on the inside walls and top. Pointed out to the PIC.
- Observed cold prep table attempting to use ice to cold hold items. Diced and sliced tomatoes as well as sour cream were at 48-56 degrees Fahrenheit. Items were to be discarded. Cold hold unit should be repaired immediately and ensure other methods are used to properly hold foods at 41 F or below. Discussed with PIC.
- Observed plastic container of sliced ham that should have been discarded on Jan. 26. (7 days prior to inspection).
- Observed a one gallon jug of a clear liquid that did not have any label at all. Recommend discarding if the contents are uncertain. Discussed with PIC.
- Observed milk that was past its use by date of Jan. 28.
- Observed a cup being used in place of a scoop in a bulk food container. Discussed with the PIC.
- A follow-up inspection is scheduled for Feb. 10.

- Active Managerial Control measures are not satisfactory for establishment to be in compliance with code. CA: Persons in charge, managers or CFSM must be properly trained or retrained. Person in charge failed to demonstrate knowledge about proper reheating techniques. CA: Discussed with manager someone getting ServeSafe certified ASAP. Must have someone registered for a class within 14 days.
- Observed soiled ice bin. CA: Discussed with employees daily cleaning.
- Food service license not properly displayed. Discussed with PIC about displaying licences.
- Observed food and debris underneath cooking equipment. CA: Discussed with PIC about cleaning under equipment more frequently.
- Personal items (phone) stored on prep table. CA: Employee removed.
- Discussed with PIC about CFSM. She explained that she had completed CFSM class and will have the certificate available. Will return to facility in 7 days to verify this.
