• Pests observed both dead and alive throughout several rooms in the facility. Observed unapproved pest control measures used in rooms 139 & 140.

• Ice machine in disrepair.
• As noted on previous inspection, lights not shielded in restrooms.
• As noted on previous inspection report, tubs stained and in need on cleaning in multiple rooms.
• Ventilation system in multiple bathrooms not in working condition. Mold found in multiple areas of the facility. Worst condition found in occupied rooms 144 & 141.
• As noted on previous inspection, floors, walls, ceilings appear dirty with graffiti on the walls.
• Observed unapproved cooking equipment in rooms 101, 125, 126, 128, 131, 141, 144.
• As noted on previous inspection, Furniture, carpet dirty and in significant need of cleaning or being replaced.
• As noted in previous inspection, Bed sheets, pads and coverings observed with stains, hair and food particles in them.

• Observed fish in a plastic bag in the freezer that supposedly had been caught from a local lake by a friend. The fish was discarded during the inspection.
• Observed several storage bins of cooked foods in the kitchen and walk in cooler that were not covered properly. All foods were covered during the inspection.
• Observed cooked pork in the walk in cooler that did not have any date markings on it as to the cook date or discard date. Discussed with PIC.
• Observed cooking equipment and food cold storage and hot holding equipment that had excessive grease and food debris accumulations especially on the handles that needs to be scrubbed and cleaned completely. May also need to repaint the white refrigerator doors to provide a light colored surface that can be maintained visually clean.
