• Observed raw chicken, beef, and pasta being held at a temperature of 44 degrees in large reach in cooler. CA: PIC was notified and food was thrown out food.
• Observed cold holding equipment (large reach in fridge) incapable of maintaining potentially hazardous food at proper temperatures. CA: Food was thrown out and PIC advised me that a repair man was on the way to fix the problem.
• Will write a plan of correction for the cold holding issues the facility has been consistently having. and will check unit on 11/07/17. Will advise PIC to record temperatures and monitor cold holding unit. If the unit fails again will.

• Observed live flies in kitchen. CA: Discussed methods of controlling flies with PIC.

• Observed soiled tongs with clean dishes. CA: dishes were rewashed.
• As observed in previous inspection, employees not consistent with date marking procedures. Majority of items in the walk in fridge, including bins of rice mentioned in the last inspect, were not date marked, nor did staff know the date or time items were put in the fridge. PIC was notified of the issue and discussed how to properly mark foods in storage. Rice was discarded.
