  • At time of inspection observed meat in container in sink in main kitchen. Advised PIC that current process is not an approved thawing process. Advised PIC to relocate.
  • At time of inspection observed raw chicken stored over trash can. Advised PIC to discard to prevent contamination of food product.
  • At time of inspection observed utensils/equipment not properly cleaned or stored (wet stacking). Advised PIC to reclean and not to stack items until properly dried.
  • At time of inspection observed heavy build up and black substance on cutting surfaces on boards near (pt) on front line and near ventilation system in main kitchen. Advised PIC to discard to prevent contamination.
  • At time of inspection observed ceiling tile missing over/near ventilation hood. Advised to replace prevent harborage.

  • Observed pesticide utilized in main kitchen. COS: Informed PIC that only a licensed pest contractor may distribute pesticide in a food service facility.
  • Observed failure to store food at least 6 inches above the ground in walk in cooler and in dry storage area. COS: Advised PIC to remove food from the floor area.
  • Observed failure to obtain and post a valid food service permit.
  • Observed failure to post must current health inspection score for public view.
  • Observed failure to provide test strips upon request.
  • Observed build-up of food, grease, and dirt debris build up on surfaces of the facility. COS: Informed facility that the cleaning frequency must be increased.
  • Observed heavy food debris in RIC doors, floors, and gaskets. COS: Informed PIC that the cleaning frequency must be increased.
  • Observed mop can wash sink in disrepair. Constant leak at faucet.
  • Observed food, grease, and dirt build up on floor, walls, and ceilings throughout the facility. COS: Informed PIC that the facility requires a detailed cleaning.
  • Observed failure to provide ceiling tile above ice machine.
  • Observed heavy fly activity in main kitchen. COS: Advised PIC to take the action needed that is listed on the pest control invoice. Corrected On-Site.
  • A follow-up inspection will be held, but no date was provided

  • Observed failure to properly date mark TCS food items stored in reach in cooler longer than 24 hours. Except when packaging food using a reduced oxygen packaging method and except as specified in paragraphs 4 and 5 of this subsection, refrigerated, ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days. The day of preparation shall be counted as Day 1. Advised PIC to properly label all food stored until next day with date or prep as day 1.
  • Observed failure to label containers of spices stored outside of its original container. Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food. Advised PIC to label all food containers and spices stored outside of its original container with name of item.
  • Observed food stored in dry storage closet directly on floor. All items must be at least 6 inches above ground. Advised PIC to rearrange dry storage and remove items from ground.
  • Observed failure to wear a hair restrain while preparing food in the main kitchen area. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. Advised PIC to purchase hair nets.
  • Observed expired food service permit posted within plain view of customers. A food establishment must have a up to date permit posted. Legal notice issued to contact Fiscal Service.
  • Observed failure to provide chlorine test strips for chemical sanitizer used for mechanical dishwasher. Advised PIC to purchase test strips for all chemicals used in main kitchen to sanitize or clean surfaces.

  • In WIF observed raw meats and partially raw meats stored above ready to eat foods. Always store raw items below, and separated from, ready to eat items.
  • Observed items on salad bar station not held 41 degrees or lower as required. Please ensure that adequate measures are taken to hold food cold. Ice packs are not capable of maintaining large containers of food cold as the pack only chills what it touches. No item on the bar was cold enough because one ice pack is not sufficient. If an electronically controlled refrigerated bar is not available then consider using smaller containers with ice baths. The food container must be submerged fully down into the ice and the ice must come up as high as the food is in the container. A new method for holding food cold on the salad bar must be used immediately.
  • Although hot box thermometer read 150F observed chicken strips in the box at 119 degrees. All hot food must be maintained at 135 degrees or higher.
  • Observed that none of the dumpsters outside have the required lids/doors. Dumpsters must remain closed/covered at all times. Either provide the doors/lids or replace the containers.

Food employees shall minimize bare hand and arm contact with exposed food that is not in a ready-to-eat form.
  • Observed failure to provide cup with lid and straw at front food service line. COS. Cup removed from prep line and thrown away.
  • Observed old inspection score posted within plain view of customer during the time of inspection. Current food service inspection must be posted at all times.
  • Observed failure to properly air dry clean dishes stacked (wet nesting) on dish rack located beside 3 compartment sink. Must advise that dishes are properly air dried before stacking.
