Deficiencies observed: Hanging thermometer in refrigeration unit at alzheimers' dining area was unavailable; in-use, wet wiping cloths not stored in sanitizing solution; dish-drying rack had chipped paint; dishes had final rinse in machine at wrong temperature; Dumpster not equipped with drain plug; and dish washing area missing two wall tiles.

Deficiencies observed: Handles of utensils at the hibachi grill stored in a way that would contaminate food items; employee rinsing raw shrimp and storing raw fish at the vegetable sink; raw beef stored above cooked ribs and corn-on-the-cob; raw egg shells stored above baby corn and washed onions; mixture of raw beef and raw chicken at wrong temperatures at hibachi grill (too low of a temperature causes bacteria contamination); and cooked potentially hazardous foods cooled to wrong temperature.  

Additional notes for this location: Raw beef was offered as well-done; one opened can of sauce was in wrong container; one container of food stored on the floor; and one container of butane was being stored with food (bananas).

Deficiencies observed: Food safety manager’s certificate posted in the facility expired on 7-18-2011 and employee wearing unapproved jewelry.

Location: 3550 Centerville Highway
Score: 87/B
Previous Scores: 96/A (Feb. 11, 2011), 91/A (Oct. 5, 2010)
Deficiencies observed: Employee did not wash hands in between handling raw meat and putting on new gloves, and several containers of residential use pesticides stored in the chemical storage area and under food prep tables.
