Observations: Observed employee on line not washing hands in between glove exchange. There were multiple unwashed vegetables above ready to eat food in both walk in coolers. In freezer raw chicken country fried steak was stored above ready to eat food items. In the cooler closest to the salad station unwashed tomatoes and strawberries stored above ready to eat items. In the walk in cooler there were items not covered or protected (deli meat, cheese, small amount of beef). The slicer, Sunkist machine, whipping cream mixer, and can opener had a build up of food debris. There is a pink slime and a mold like substance found in the ice machine in the kitchen. Wet wiping cloths were stored in the kitchen lying on counters. Knives that were cleaned previously were sitting in stagnant dirty water. Food containers were cracked and broken on the ends. Shelving was observed with rust and dust debris. Some kitchen equipment was being held up by pans and held together by zip ties and ducktape. Equipment is in disrepair. Observed sinks being stopped up for use by miscellaneous lids. A large cart that was being used to transport food to the line and back had a build up of debris. Kitchen equipment was observed being held up by pans, zip ties, and duck tape. Several floor tiles throughout the kitchen are broken into small and large pieces that are scattered amongst the facility. Hose was connected to the mop sink without a proper backflow device. Floor tiles in disrepair and miscellaneous placed throughout facility. The light shield in the kitchen is missing. Two employees had their phones on food contact surfaces.
